The Effects of Multiple Continuous Nursing Model on Patients with Chronic Heart Failure

Abstract:Objective:To explore the effects of multiple continuous nursing care model on self -care ability and readmission rate of pa-tients with chronic heart failure (CHF).Methods:According to the draw method ,a total of 128 cases of CHF patients were randomly divided into the observation group and the control group with 64 cases in each group .The control group received routine rehabilitation nursing after discharge ,while the observation group received the multiple continuous nursing model based on the control group .The incidence of cardio-vascular events ,readmission rate ,mortality rate and intervention before and after the self -care ability and quality of life scores were com-pared between two groups .Results:The observation group′s incidence of cardiovascular events and readmission rates were significantly low -er than those in the control group(P<0.05),and the observation group after the intervention self -care ability and life quality scores were significantly higher than those in the control group (P<0.05).Conclusion:The multiple continuous nursing model can effectively improve the self-care ability of CHF patients ,reduce the incidence of cardiovascular events and readmission rate ,and thus improve the quality of life.
