Application of chronic disease management model in patients with rheumatoid arthritis

Abstract：Objective:To investigate the application effect of chronic disease management model in patients with rheumatoid arthritis. Methods:60 patients with rheumatoid arthritis were randomly divided into the experimental group(n = 23)and the control group(n = 37). The routine nursing care for patients with rheumatoid arthritis was taken in the control group;the chronic disease management model was used to conduct nursing intervention in the experimental group. The quality of life,awareness of the related knowledge of the disease and the satisfaction rate of the patients were compared between the two groups. Results:The scores of the quality of life of the patients in the ex-perimental group were lower than those in the control group(P < 0. 05);the awareness of the related knowledge of the disease and the sat-isfaction rate of the patients were better and higher in the experimental group than the control group(P < 0. 05). Conclusion:The nursing intervention in the light of chronic disease management model can effectively improve the quality of life and satisfaction rate of the patients with rheumatoid arthritis.
