Application of transitional care model in patients with enterostomy

Abstract：Objective:To explore the application method and effect of transitional care model in patients with enterostomy. Methods:20 patients who underwent enterostomy from January 2009 to December 2010 were taken as the control group and the patients in this group were given routine health guidance;other 23 patients who received enterostomy from January 2011 to December 2012 were taken as the ob-servation group and the patients in this group were provided extra transitional care model. The indexes like self - care ability,defecation regularity,complications,sexual function and attitudes towards life of the patients were compared between the two groups. Results:There were statistically significant differences in the comparison of self - care ability,defecation regularity,complications and attitudes towards life of the patients between the two groups(P ﹤ 0. 05). Conclusion:The transitional care model can improve the self - care ability of patients with enterostomy,reduce the complications and improve the quality of nursing care.
