Influence of Laparoscopic mini-cholecystolithotomy with gal bladder preserved on circulatory and respiratory system of the surgical patients and the nursing countermeasures

Abstract：Objective:To investigate the influence of Laparoscopic mini -cholecystolithotomy with gallbladder preserved on circulatory and respiratory system of the surgical patients and the nursing countermeasures .Methods:96 patients with gallstone were randomly divided into the control group and the observation group (49 cases in each group).The pneumoperitoneum pressure of the patients in the control group was 13~15 mm Hg(1 mm Hg=0.133 kPa),and the routine nursing care was given to the patients till pneumoperitoneum pressure was constant;the pneumoperitoneum pressure of the patients in the observation group was 8~12 mm Hg,and the pneumoperitoneum pres-sure was intermittently adjusted till it was constant ,then the corresponding nursing measures was implemented .BP HR and SpO2 were re-spectively observed in the two groups before pneumoperitoneum ,2 and 10 minutes after pneumoperitoneum and after deflation .The postop-erative recovery of the patients was compared between the two groups .Results:SBP,HR and SpO2 decreased to some extent in 2 and 10 mi-nutes after pneumoperitoneum ,compared with those in the control group (P<0.05);the significant change was found in SBP ,HR and SpO2 only in 2 minutes after pneumoperitoneum in the observation group (P<0.05);SBP,HR and SpO2 were lower in the control group than the observation group in 2 and 10 minutes after pneumoperitoneum (P<0.05);the recovery time of bowel sound was significantly shorter and the dosage of analgesic was significantly lower in the observation group than the control group (P<0.05).Conclusion:In lapa-roscopic operation,the high pneumoperitoneum pressure has the obvious influence on respiratory and circulatory system of the patients .So the continuous high pneumoperitoneum pressure should be avoided in operation and the pertinent nursing care be strengthened in order to maintain the stability of the function of respiratory and circulatory system of the patients .
