Abstract:Esketamine,as the S(+)-enantiomer of ketamine, possesses unique pharmacological properties, enabling rapid and effi-cient analgesic and sedative effects through NMDA receptor blockade. In short-duration painless procedures such as gastroscopy, abortion and bronchoscopy, the combined use of esketamine with propofol or remimazolam has optimized anesthesia protocols by reducing the inci-dence of hypoxemia, hypotension, and postoperative adverse reactions, thereby improving patient comfort and recovery quality. Its rapid metabolism significantly shortens postoperative recovery time and enhances operating room efficiency. However, potential impacts during high-dose or long-term use require attention. This article summarizes the pharmacological mechanisms and current clinical applications of esketamine, proposes strategies for personalized use in high-risk patients and highlights its significant advantages and development poten-tial in the field of painless procedures.
