Effect of dexmedetomidine on the incidence of postoperative delirium in elderly patients undergoing joint arthroplasty:a meta-analysis

Abstract:Objective To systematically evaluate the effect of dexmedetomidine on the incidence of postoperative delirium in elderly pa-tients undergoing joint arthroplasty.Methods A comprehensive literature search was conducted for relevant studies published from Janu-ary 1st,2013 to October 31st,2024 in CNKI,PubMed,Embase,Cochrane Library,Wanfang Database,Medline,and VIP databases,yielding 166 studies.After screening,11 studies were included in the Meta-analysis.The primary outcomes evaluated were the incidence of postoperative delirium,nausea and vomiting,hypertension,bradycardia and hypotension.Results A total of 11 randomized controlled trials with 1785 patients were included,889 patients in dexmedetomidine group,896 patients in the control group.Compared with the control group,the incidence of postoperative delirium in dexmedetomidine group was significantly lower(OR=0.40,95%CI:0.30-0.52,P<0.001).The risk of hypertension was also significantly lower(OR=0.48,95%CI:0.24-0.97,P=0.04).The differences in the incidence of postoperative nausea and vomiting,bradycardia and hypotension were not statistically significant(P>0.05).Sub-group analysis revealed no significant difference in the effect of dexmedetomidine on postoperative delirium when different control drugs or anesthetic techniques were used(P>0.05).Conclusion The perioperative use of dexmedetomidine reduces the risk of postoperative delirium in elderly patients undergoing various types of joint arthroplasty.
