Comparison of analgesic effects between suprascapular nerve block combined with erector spinae plane block and brachial plexus block combined with thoracic para-vertebral block in patients with scapular fracture

Abstract:Objective To compare the analgesic effects of suprascapular nerve block(SNB)combined with erector spinae plane block(ESPB)and brachial plexus block(BPB)combined with thoracic paravertebral block(TPVB)in patients with scapular fracture.Methods Forty patients of either gender,aged 35-76 years,BMI 51-85 kg,ASA Ⅰ or Ⅱ,scheduled for unilateral scapular fracture surgery via the Judet approach at Zibo 148 Hospital from January to August 2024 were enrolled.Patients were randomly divided into two groups,ultrasound-guided SNB combined with ESPB(group S,n=20)and ultrasound-guided BPB combined with TPVB(group C,n=20).After general anesthesia,SNB and ESPB were performed in group S,and BPB and TPVB were performed in group C.The time required for regional block operation,intraoperative consumption of propofol and remifentanil,the resting and active VAS scores at 2,6,12,24,and 48 h after surgery,the number of PCIA compressions,the number of cases receiving tramadol remedies for analgesia,the satisfaction of postoperative analgesia effect,the occurrence of nausea,vomiting,skin prurities and respiratory depression were recorded in both groups.Results The time required for nerve block operation in group S was shorter than that in group C,with statistical significance(P<0.05).There were no statistically significant differences in propofol and remifentanil consumption,VAS scores at rest and activity at each time point,PCIA compression times,tramadol relief analgesia cases,postoperative analgesic satisfaction,nausea and vomiting between the two groups(P>0.05).There were two cases of chest tightness in group C.Conclusion Both SNB combined with ESPB and BPB combined with TPVB can provide good analgesia in patients with scapular fracture.However,SNB combined with ESPB is simp-ler and safer.
