Infulencing factors of preoperative sleep disorders in patients undergoing laparo-scopic cholecystectomy

Abstract:Objective To investigate the status of preoperative sleep disorder and its influencing factors in patients undergoing laparo-scopic cholecystectomy(LC).Methods Basic clinical data of 97 patients who underwent elective LC in the First Affiliated Hospital of Hainan Medical University from May 2022 to October 2023 were collected.The Pittsburgh Sleep Quality Index(PSQI)was used to evalu-ate the preoperative sleep quality of the patients,and according to the PSQI score,patients who scored<8 were included into the normal sleep group(n=49),and patients whose PSQI score≥8 were included into the sleep disorder group(n=48).The self-made question-naire was used to record and compare the general information and clinical data of both groups.x2 and rank sum test was used to compare the influencing factors of preoperative sleep disorder in the two groups;Logistic regression analysis was applied to explore the independent risk factors affecting preoperative sleep disorder in LC patients.Results Among the 97 patients in the study,50.52%patients were included in the normal sleep group and 49.48%were included in the sleep disorder group.Results of univariate analysis:indicators including difficulties in falling asleep due to unfamiliar environment and doctor's check-ups,fear of prognosis,anxiety,gallbladder pain,difficulties in hospitalisation costs and underlying diseases were associated with preoperative sleep disorder in LC patients(P<0.05).Logistic analysis showed that sleep was affected by unfamiliar environment and doctor's check-ups(OR=4.792,95%CI=1.494-15.367),anxiety(OR=2.969,95%CI=1.322-15.044),gallbladder pain(OR=6.932,95%CI=2.159-22.254),and difficul-ties in hospitalisation costs(OR=4.863,95%CI=1.474-15.869).Conclusion Preoperative sleep disorder in LC patients is associ-ated with a variety of factors such as unfamiliar environment,doctor's visit,anxiety,gallbladder pain,and difficulties in hospitalisation costs,and these influencing factors should be given preoperative interventions to improve sleep quality.
