Abstract：With the accelerating trend of population aging in China,the prevalence of chronic diseases has been steadily increasing.In response to these demographic challenges,the healthcare service system has prioritized proactive health management as a key strategy for chronic disease prevention and control.Driven by the rapid advancement of digital and intelligent technologies,digital intelligence empowering proactive health management has broken through the limita-tions of traditional models.By accurately identifying individual characteristics and health needs,it enables population health stratification and supports the development of personalized management plans.It has become an important develop-ment direction for chronic disease management.This paper aims to provide an overview of the digital intelligence-empow-ered proactive health management model,systematically summarize the key technologies,implementation pathways,and practical outcomes,and analyze existing challenges and future prospects,with the goal of offering a reference for enhanc-ing the quality of chronic disease health management in China.
