Clinical Manifestation and Ultrastructure of 16 Patients with Seasonal Acquired Symmetric Acrokeratoderma

Abstract:Objective:To analyze the clinical data of 16 patients with seasonal acquired sym-metric acrokeratoderma and summarize its features of clinical manlfestation and ultrastructure . Methods:16 patients in our hospital from March 2010 to November 2012 were followed up and their clinical data were collected and analyzed in the study .Results:There were 11 male and 5 fe-male patients among these 16 patients.The mean age was (28.3 ±2.1) years old and the age at onset was (19.5 ±1.2) years old.Lesions usually occurred or relapsed in the end of spring or the beginning of summer , most of cases were aggravated in summer ,and vanished or alleviated in the end of autumn or the beginning of winter .The lesions usually symmetrically distributed on the both of wrists,the back of hands,ankles,knees,dorsum of feet,elbows,finger webs,lateral margin of hands and triangles of elbow .All lesions showed the appearance of pigmentation ,lichenoid ,hyper-keratotic patches or plaques and without noticeable discomfort on the lesions .KOH tests were neg-ative.The histopathology revealed hyperkeratosis and papillomatosis of the epidermis .Ultrastructure showed cutin layer upon and increased in the layer number , keratinization coated poor continuity , keratin filaments was disorder in structure into tear .The surrounding granular layer and upper spi-nous layer cells , some keratin filaments were visible aggregation abnormally .Conclusion:16 patients with seasonal acquired symmetric acrokeratoderma present obvious seasonal characteristics .Histopa-thology and ultrastructure findings are similar to congenital bullous ichthyosiform erythroderma .
