An overview of the mechanism of action of isatuximab in the treatment of multiple myeloma

Abstract:Multiple myeloma(MM)is the second most common hematological malignancy and remains incurable to date.As China's population ages at an accelerating pace,the disease burden associated with MM continues to grow.CD38 has emerged as a key therapeutic target in MM,and anti-CD38 monoclonal antibodies have progressively become cornerstone treatments in its management.As a novel anti-CD38 monoclonal antibody approved in China in early 2025,isatuximab currently has limited real-world clinical experience,and its mechanism of action warrants further elucidation.A deeper understanding of the differences in the mechanisms of action among various anti-CD38 antibodies may enhance the comprehension of this drug class and offer valuable insights for clinical decision-making.Among currently available anti-CD38 monoclonal antibodies,isatuximab and daratumumab are the two agents with the most substantial and systematically documented mechanistic and clinical evidence.Therefore,this review employs daratumumab as a key comparator to systematically outline the mechanistic features of isatuximab in the treatment of MM and to highlight its distinctions from other anti-CD38 monoclonal antibodies,with the goal of providing a theoretical foundation for individualized MM therapy.
