Research status and progress of poorly cohesive gastric carcinoma

Abstract:Poorly cohesive gastric carcinoma(PC-GC)is a special pathological type of gastric cancer with highly malignant characteristics.Its typical histological manifestations are diffuse growth of tumor cells,accumulation of intracellular mucus and absence of intercellular adhesion.In most cases,it presents as intramucosal carcinoma.Recent epidemiological data show that the incidence of PC-GC has shown a significant upward trend,posing a serious threat to the survival of patients.At present,the specific treatment strategies for PC-GC are not yet perfect and remain an important issue that urgently needs to be broken through in the field of clinical oncology.This article systematically reviews the latest research progress of PC-GC,covering histological classification evolution,auxiliary diagnostic techniques,molecular characteristics and treatment strategies.Studies have shown that endoscopic resection can be an important treatment option for early undifferentiated PC-GC.For advanced cases,total gastrectomy or subtotal gastrectomy still has certain clinical value.Although adjuvant chemotherapy plays a role in the treatment of PC-GC,this subtype has a relatively low sensitivity to chemotherapy,and its survival rate and prognosis are significantly worse than those of non-PC-GC gastric cancer.Molecular studies have shown that genes such as FGFR2,CDH1 and RHOA may become potential therapeutic targets.This review provides a theoretical basis for the clinical diagnosis and treatment decisions of PC-GC and offers an important reference for future research directions.
