Superficial cervicovaginal myofibroblastoma:a clinicopathological analysis of sixteen cases

Abstract:Objective To investigate the clinical and pathological characteristics,diagnosis,differential diagnosis and prognosis of superficial cervicovag myofibroblastoma(SCVMF).Methods A total of 16 cases of SCVMF confirmed by the Department of Pathology,Wuhan Central Hospital from January 2015 to October 2024 were collected,and their clinical and pathological data were retrospectively analyzed and summarized.Results The age of the 16 patients ranged from 40 to 72 years(average age 54.5 years);7 cases were menopausal patients,and 9 cases were postmenopausal patients.The duration of the disease ranged from 4 days to 5 years.All were solitary,11 cases were located in the vagina,and 5 cases were located in the cervix.Six cases presented with irregular vaginal bleeding,2 cases with difficulty in urination,and the remaining 8 cases had no obvious symptoms.Grossly,the tumors were nodular or polypoid,with smooth surface,ranging in size from 1.5 to 5 cm in diameter,with a grayish-white,soft,translucent or mucoid cut surface.Histologically,the tumors were located in the submucosa,without a distinct capsule but with clear boundaries.The tumors had a moderate cell density,with sparse cells in the superficial areas and relatively dense cells in the deeper areas.The tumors exhibited various architectural patterns:reticular,sieve-like,short fascicular,randomly distributed.The stroma showed fine collagen,myxoid edema,or focal hyaline degeneration,with scattered thin-walled vessels,small medium-sized lumens,and varying numbers of mast cells and lymphocytes.Immunohistochemically,the tumor cells were positive for vimentin(16/16),desmin(16/16),ER(16/16),PR(16/16)and CD34(14/16);4 cases were focally positive for SMA;S-100 and CK were negative.All 16 cases were up for 1 month to 8 years and 5 months(average 39 months),and none of them showed recurrence.Conclusion SCVMF is a benign mesenchymal tumor arising from the superficial stroma beneath the mucosa of the female cervix and vagina.It can be easily confused with fibroithelial polyps,angiomyofibroblastoma,and invasive angioleiomyoma,and a definite diagnosis requires a combination of pathological morphology,ohistochemistry,and clinical data.
