Predictive value of the CBLC gene for prognosis and immunotherapy efficacy in lung adenocarcinoma

Abstract:Objective To investigate the expression level of CBLC gene in lung adenocarcinoma and its predictive value for prognosis and immunotherapy effect.Methods Gene expression data and clinical information of 516 lung adenocarcinoma patients were collected from the cancer genome atlas(TCGA)and GTEx databases,and the expression level of CBLC gene in lung adenocarcinoma tissues and cell lines were verified by quantitative real-time PCR(qRT-PCR).Logistic regression analysis was employed to evaluate the correlation between CBLC expression and clinicopathological features.The relationship between CBLC gene expression levels and patient outcome was assessed using a Kaplan-Meier survival analysis and Cox regression model.Single-sample gene set enrichment analysis(ssGSEA)was used to analyze the correlation of CBLC gene expression with the level of immune infiltration.Results CBLC gene expression was significantly higher in lung adenocarcinoma tissues and lung adenocarcinoma cells than in adjacent tissues and normal alveolar epithelial cells(P<0.05).In addition,CBLC gene expression was significantly associated with pathological T stage(OR=0.509,95%CI:0.300-0.865,P=0.013)and smoking history(OR=2.422,95%CI:1.452-4.040,P<0.001).Kaplan-Meier survival analysis showed that overall survival(HR=1.510,95%CI:1.130-2.020,P=0.005)and disease-specific survival(HR=1.630,95%CI:1.130-2.360,P=0.009)of CBLC high-expression patients were significantly lower than those of CBLC low-expression patients.Univariate Cox regression analysis revealed that patients with pathological T3/T4(HR=2.352,95%CI:1.614-3.426,P<0.001),N2/N3(HR=2.360,95%CI:1.659-3.358,P<0.001),and M1(HR=2.176,95%CI:1.272-3.722,P=0.005)had a poorer prognosis.Multivariate Cox regression analysis indicated that pathological T stage(HR=1.670,95%CI:0.921-3.027,P=0.091),N stage(HR=2.075,95%CI:0.880-4.893,P=0.095),and M stage(HR=1.496,95%CI:0.576-3.891,P=0.408)were not significantly associated with prognosis.Immune infiltration analysis showed that CBLC gene expression was inversely associated with infiltration levels of Th1(r=-0.240,P<0.001),helper T cells(r=-0.242,P<0.001)and macrophages(r=-0.272,P<0.001).Conclusion CBLC gene is highly expressed in lung adenocarcinoma and is associated with prognosis and immune infiltration level,which may serve as potential biomarkers for prognosis and immunotherapy effect in lung adenocarcinoma.
