Research progress in neoadjuvant and adjuvant therapy for malignant biliary tract cancer

Abstract:Biliary tract cancers(BTC)are a group of highly malignant tumors with poor prognosis,including intrahepatic and extrahepatic bile duct cancer and gallbladder cancer.Due to the nonspecific early symptoms and special anatomical location,most patients have lost the optimal timing for surgery by the time they seek medical attention,resulting in a low surgical resection rate.Even after surgical resection,the risk of postoperative recurrence is as high as 60%-70%.Against this backdrop,neoadjuvant therapy and adjuvant therapy have emerged.Neoadjuvant therapy refers to systemic anti-tumor treatment given to patients before surgery,aiming to reduce tumor volume,improve surgical resection rates,and prolong disease-free survival;adjuvant therapy is systematic treatment given to patients after surgery,aimed at eradicating residual tumor foci and preventing recurrence.Currently,neoadjuvant and adjuvant treatments include chemotherapy,molecular targeted therapy,immunotherapy,etc.,some therapies can also be used in combination with local treatments(such as radiotherapy,embolization,etc.),hoping to improve the prognosis of patients with biliary tract tumors through synergistic effects in multiple ways.This article reviews the current status,research progress,and future development directions of neoadjuvant and adjuvant treatments for biliary tract tumors,providing new ideas for improving the efficacy of these patients.
