Analysis of influencing factors for the occurrence of radiation sinusitis in nasopharyngeal carcinoma patients undergoing radiotherapy

Abstract:Objective To investigate the risk factors of radiation sinusitis in patients with nasopharyngeal carcinoma.Methods The clinical data of 120 patients with nasopharyngeal carcinoma admitted to Tangshan Central Hospital and Tangshan People's Hospital from June 2019 to June 2023 were retrospectively analyzed.All patients received intensity-modulated radiotherapy.The incidence of radioactive sinusitis in patients with nasopharyngeal carcinoma radiotherapy was recorded,and the factors of occurrence of radioactive sinusitis in patients with nasopharyngeal carcinoma radiotherapy were analyzed.Multivariate Logistic regression model was used to analyze the high-risk factors of radioactive sinusitis in patients with nasopharyngeal carcinoma radiotherapy.Results Among 120 patients with nasopharyngeal carcinoma,71 cases(59.17%)developed sinusitis.Univariate analysis showed that duration≥2 years,clinical stage T3+T4,nasal invasion,radiation dose>60 Gy,use of nasal drops and nasal irrigation were the factors affecting the incidence of radiation sinusitis in nasopharyngeal carcinoma radiotherapy patients(P<0.05).Multivariate Logistic regression model showed that duration≥2 years(OR=4.754),clinical stage T3+T4(OR=3.057),nasal invasion(OR=34.021)and radiation dose>60 Gy(OR=4.284)were risk factors for radiation sinusitis(P<0.05).The use of nasal drops(OR=0.162)and nasal irrigation(OR=0.232)were protective factors for radiation sinusitis(P<0.05).Conclusion Radioactive sinusitis occurs in patients with nasopharyngeal carcinoma treated with radiotherapy,and its occurrence is related to the course of disease,clinical stage,nasal invasion,radiation dose,use of nasal drops and nasal irrigation,etc.Therefore,targeted preventive measures can be developed to prevent radioactive sinusitis in patients with nasopharyngeal carcinoma treated with radiotherapy.
