Relationship between blood flow parameters and microvessel density in cervical cancer with Doppler ultrasound

Abstract:Objective To analyze the correlation between blood flow parameters measured by Doppler ultrasound and microvessel density(MVD)in cervical cancer patients.Methods The clinical data of 95 cervical cancer patients(cervical cancer group)admitted to Changzhi People's Hospital from July 2022 to November 2024 were retrospectively collected.Meanwhile,90 patients diagnosed with benign cervical lesions by pathological examination were selected as the control group.Ultrasound image features and blood flow parameters measured by ultrasound[resistance index(RI),peak systolic velocity(PSV)and end diastolic velocity(EDV)]were compared between the two groups.RI,PSV,EDV,and MVD in patients with different stages of cervical cancer were compared.The correlation between RI,PSV,EDV and MVD was analyzed.Results Among the 95 patients with cervical cancer,there were 23 in stage I,37 in stage II,22 in stage III,and 13 in stage IV.There were 71 cases of squamous cell carcinoma and 24 cases of adenocarcinoma.The proportions of cervical enlargement,hyperecho,and hypoecho in the cervical cancer group were higher than those in the control group(all P<0.05).RI in the cervical cancer group(0.99±0.25)was lower than that in the control group(1.59±0.43)(P<0.05).PSV,EDV and MVD[(37.42±10.33)cm/s,(10.56±2.72)cm/s and(24.46±6.94)/mm2]were higher than those in the control group[(24.50±7.98)cm/s,(5.53±1.31)cm/s and(13.10±3.32)/mm2](P<0.05).There were statistically significant differences in RI,PSV,EDV,and MVD among patients with different stages of cervical cancer(P<0 05).With the increase of stage,RI decreased[(1.16±0.23),(1.05±0.22),(0.84±0.19)and(0.72±0.16)],while PSV[(28.65±7.94)cm/s,(35.47±8.12)cm/s,(44.59±8.13)cm/s and(46.38±9.93)cm/s],EDV[(8.63±2.19)cm/s,(10.47±2.23)cm/s,(11.36±2.30)cm/s and(12.85±3.11)cm/s]and MVD[(17.23±4.68)/mm2,(24.36±5.19)/mm2,(28.74±5.86)/mm2 and(30.26±6.50)/mm2]increased.There were no statistically significant differences in RI,PSV,EDV and MVD between patients with squamous cell carcinoma and those with adenocarcinoma(P>0.05).Pearson analysis results showed that RI was significantly negatively correlated with MVD in cervical cancer patients(r=-0.639,-0.715,both P<0.05),while PSV and EDV were significantly positively correlated with MVD[(r=0.915,P<0.001),(r=0.824,P<0.001);(r=0.803,P<0.001),(r=0.746,P<0.001)].Conclusion Blood flow parameters measured by Doppler ultrasound can reflect the changes in blood flow in cervical cancer at different stages,and they are significantly correlated with MVD in the lesion.
