Efficacy of conventional magnetic resonance imaging in determining glioblastoma IDH1 mutation status

Abstract:Objective To analyze the efficacy of conventional magnetic resonance imaging(MRI)in the diagnosis of isocitrate dehydrogenase(IDH)1 mutation in glioblastoma(GBM).Methods Clinical medical records of 90 GBM patients admitted to Yuncheng Central Hospital from March 2020 to June 2024 were collected.T1 and T2-weighted imaging,T1 enhanced(T1+C),and T2-fluid attenuated inversion recovery(FLAIR)imaging were obtained.The tumor volume,absolute tumor blood flow(aTBF),relative tumor blood flow(rTBF),maximum cross-sectional necrotic area(NECarea),and percentage of the non-enhanced area(%NEC)within the maximum cross-sectional enhanced lesion were collected.IDH1 mutation was detected by immunohistochemical staining.Results The age,aTBF,rTBF,NECarea and%NEC of IDH1 mutant type patients were lower than those of IDH1 wild type patients,and the total tumor volume of IDH1 mutant type patients was also higher than that of IDH1 wild type patients(P<0.05).LASSO regression and multi-factor Logistics regression analysis showed that age,tumor volume and aTBF were independent clinical factors related to IDH1 mutant type.The area under the receiver characteristic curve of patients diagnosed with IDH1 mutation by age,total tumor volume and aTBF combined indexes was 0.878(95%CI:The sensitivity and specificity were 89.47%and 85.92%,respectively,and the goodness of fit was good(P=0.972).Conclusion This study found that IDH1 mutation status in GBM patients can be diagnosed by combined age with tumor volume and aTBF in conventional MRI indicators,providing a simple method for accurate,early(preoperative)and non-invasive assessment of IDH1 mutation status in GBM patients.
