A Case of Immune Checkpoint Inhibitor-Related Sarcoidosis-like Reaction
Abstract:Immune checkpoint inhibitors (ICIs) have been widely used in the treatment of various solid tumors, including brain gliomas, lung cancer, and urothelial cancer, and have brought hope to many patients with advanced tumors. However, the T-cell immunity activated by ICIs may attack normal tissues and organs, leading to immune-related toxicities. Sarcoidosis is a non-caseating granulomatous inflammation, and more than 90% of patients are involved in the chest, presenting as enlarged mediastinal or hilar lymph nodes and/or pulmonary infiltration, with an unknown cause. There are reports that drugs such as tumor necrosis factor (TNF) antagonists can cause sarcoidosis-like manifestations, defined as drug-induced sarcoidosis-like reaction (DISR). In recent years, multiple ICIs have been reported to induce DISR, but there has been no report on DISR induced by sintilimab. Herein, we report the diagnosis and treatment of a case of DISR in a patient with bladder cancer after treatment with sintilimab.
