Clinicopathological characteristics and prognosis of multiple myeloma with CD3+T lymphoid nodules

Abstract:Objective To explore the clinicopathologic features and prognosis of multiple myeloma(MM)with CD3+T lymphoid nodules.Methods The clinical data of 58 patients with MM and 58 patients with bone marrow biopsy eligible for bone marrow transplant donors from January 30,2020 to January 30,2022 were retrospectively analyzed.MM patients were further divided into the group with CD3+T lymphnoid nodules and the group without CD3+T lympoid nodules.The clinical features,pathological features,short-term efficacy and prognosis of patients in the two groups were observed.Results CD3+T lymphoid nodules were present in 33 cases(74.1%),and the proportion of CD3+T lymphocytes was>5%in 74.1%of the patients.In the control group,only 13.3%of the donors had CD3+T lymphoid nodules,and the proportion of CD3+T lymphocytes was<5%in 65.0%of the donors.The presence of CD3+T lymphoid nodules was associated with the proportion of CD3+T lymphocytes,CD20+B lymphocytes,Ki-67 proliferation index and tumor cell burden in MM(P<0.05),but not with pathological morphology,bone marrow hyperplasia degree,CD20+B lymphoid nodules,infiltration pattern and CD56 expression(P>0.05).The treatment response rate of MM patients with CD3+T lymphoid nodules was 69.7%,which was higher than 40.0%of patients without CD3+T lymphoid nodules,with statistical significance(P<0.05).There was a statistical difference in the proportion of CD3+T lymphocytes in bone marrow biopsy after treatment between the effective group and the ineffective group(P<0.05),and there was a statistically significant difference in the proportion of CD4+T lymphocytes and CD8+T lymphocytes in peripheral blood after treatment(P<0.05).The 2-year survival rate of MM patients with CD3+T lymphoid nodules was 93.9%,which was higher than 72.0%of patients without CD3+T lymphoid nodules,and the difference was statistically significant(P=0.018).Conclusion The presence of CD3+T lymphoid nodules and an increased proportion of CD3+T lymphocytes can affect the survival of MM patients.MM patients with CD3+T lymphoid nodules have a better prognosis.
