Value analysis of the fraction of genome altered in prognostic evaluation of pancreatic ductal adenocarcinoma

Abstract:Objective To explore and analyze the impact of the fraction of genome altered(FGA)on the prognosis of pancreatic ductal adenocarcinoma(PDAC).Methods A total of 2261 PDAC patients from the MSK-IMPACT platform database were included.Twenty variables were considered.A total of 15 demographic and clinical characteristics variables,5 gene variation related indicators and 2 derived variables were included,including age,age,history of immune disease,history of coronary artery disease,history of diabetes,history of hypertension,history of pancreatitis,history of tumor,tobacco exposure,tumor site,disease status,disease stage,new adjuvant treatment,excision samples,adjuvant treatment,tumor mutation load(TMB),FGA,tumor purity,gene group,number of mutations,total survival status and total survival time.Univariate and multivariate Cox regression analysis and multivariate Lasso-Cox regression analysis were employed.Results The median FGA(interquartile range)was 0.04(0.003-0.17)in 2261 PDAC patients,who were divided into a low proportion group of 1120(49.54%)and a high proportion group of 1141(50.46%)according to the median FGA.Survival data showed that the median survival time of patients in the high FGA group was 14.2(95%CI:13.2-15.2)months,lower than that of the low FGA group of 21.0(95%CI:19.6-22.4)months(P<0.001).The rates of neoadjuvant therapy,tumor resection,and adjuvant therapy in different FGA groups were higher in the low FGA group.The results of multivariate Cox regression showed that high FGA had a poorer prognosis compared to low FGA in PDAC(HR=1.422,95%CI:1.258-1.609,P<0.001).Additionally,multivariate Lasso-Cox regression analysis indicated that high FGA was a risk factor for prognosis in PDAC patients,with a coefficient of 0.0029.Conclusion The overall survival for PDAC patients is poor.FGA is associated with PDAC tumor location,disease status,neoadjuvant therapy,tumor resection,adjuvant therapy,and other factors.It also serves as an independent risk factor for the prognosis of PDAC.
