Effect of differentiation,lymph node metastasis and adjuvant chemotherapy on prognosis of patients with duodenal adenocarcinoma after radical surgery

Abstract:Objective To investigate the effect of differentiation,lymph node metastasis and adjuvant chemotherapy on prognosis of patients with duodenal adenocarcinoma after radical surgery.Methods Clinical data of 56 patients with duodenal adenocarcinoma confirmed by histopathology from September 2008 to December 2019 were collected.Kaplan-Meier and Log-rank methods were used for survival analysis.Cox proportional regression model was used for multivariate analysis.Results Univariate analysis showed that the depth of invasion,lymph node metastasis,differentiation,nerve invasion,pancreatic invasion and adjuvant chemotherapy were correlated with overall survival(OS)of duodenal adenocarcinoma patients(P<0.05),but age,sex,tumor size and jaundice were not correlated with OS(P>0.05).Multivariate analysis showed that the differentiation,lymph node metastasis and adjuvant chemotherapy were independent factors affecting OS of duodenal adenocarcinoma patients(P<0.05).In subgroup of without lymph node metastasis,the 1-and 3-year survival rates of patients with adjuvant chemotherapy were 100.0%and 86.9%,better than 94.4%and 61.1%of patients without adjuvant chemotherapy(P=0.009).In moderately and highly differentiated subgroup,the 1-and 3-year survival rates of patients with adjuvant chemotherapy was 100.0%and 88.9%,better than 82.4%and 64.7%of patients without adjuvant chemotherapy(P=0.01).Conclusion The differentiation,lymph node metastasis and adjuvant chemotherapy are independent factors affecting the postoperative patients with duodenal adenocarcinoma.Patients without high risk factors may benefit from adjuvant chemotherapy.
