CT imaging classification characteristics of 42 cases of immune checkpoint inhibitor-related pneumonitis

Abstract:Objective To investigate the CT imaging features of checkpoint inhibitor-related pneumonitis(CIP).Methods The chest CT images of patients with CIP were analyzed.The imaging characteristics and main points of differentiation from other pulmonary diseases were summarized.Results From September 1,2016 to December 31,2022,1354 malignant tumor patients received monotherapy or combination therapy of immune checkpoint inhibitors in the oncology department of our hospital.A total of 42 cases of CIP occurred,with an incidence rate of 3.1%(42/1354).A total of 9 patients died,with a mortality rate of 0.7%(9/1354).CIP could be divided into 5 types based on CT imaging,namely ground glass type(10 cases),patchy type(16 cases),pulmonary consolidation type(2 cases),grid type(3 cases),and mixed type(11 cases).Each type had characteristic imaging manifestations and different clinical outcomes,and needed to be distinguished from infectious pneumonia,tumor progression,radiation pneumonia,etc.Conclusion CT imaging features are helpful for the early diagnosis of CIP,predicting hormone sensitivity and prognosis.
