Observation of transurethral holmium laser en-bloc resection in the treatment of non-muscular invasive bladder cancer

Abstract:Objective To compare the clinical efficacy of transurethral holmium laser resection of bladder tumor (HoLERBT) and transurethral resection of bladder tumor (TURBT) in the treatment of non-muscle invasive bladder cancer ( NMI-BC), and furher explore the advantages of HoLERBT operation. Methods The clinical data of 106 cases with NMIBC from January 2013 to January 2016 were analyzed retrospectively. According to operation method, 106 patients were divided into HoLERBT group (n=56) and TURBT group (n=50). HoLERBT and TURBT were performed respectively. The operative time, catheter indwelling time, postoperative hospital stay, bleeding volume, obturator reflex, bladder perforation and recurrence rate were compared between both groups. Results The operation time was (27. 52 ±5. 28)min in HoLRBT group and (28. 91±5. 03) min in TURBT group (P>0. 05). The postoperative bladder irrigation time, catheter indwelling time, postoperative hospital stay and average intraoperative bleed-ing in the HoLRBT group and TURBT group were (2. 20±0. 44 vs. 4. 30±0. 54) d, (2. 57±0. 44 vs. 4. 69±0. 61) d, (3. 96±0. 54 vs. 6. 21±0. 63) d and (20. 25±3. 53 vs. 34. 82±4. 84) ml, respectively. There were significant differences between the two groups (P<0. 05). There were no obturator reflex and bladder perforation in HoLRBT group, but 12 cases of obturator reflex and 5 cases of bladder perforation in TURBT group with statistical significance ( P<0. 05). The muscle layer rate of postoperative pathological specimen was 100. 0%(56/56) of HoLERBT group, higher than 70. 0%(35/50) of TURBT group (P< 0. 05). The 2-year recurrence rate after op-eration in HoLERBT group was 14. 3%(8/56), significantly lower than 32. 0%(16/50) in TURBT group. Conclusion The clinical efficacy of HoLERBT in the treatment of NMIBC is better than TURBT and the complications are less. It is worth popularizing in clini-cal practice.
