Clinical observation of S-1 in treating elderly prostate patients after the failure of androgen deprivation therapy

Abstract:Objective To compare the curative effect of S-1 regime and docetaxel regime in treating elderly prostate patients after the failure of androgen deprivation therapy. Methods From June 2010 to July 2015, 126 cases of old patients suffering from prostate cancer were assigned to receive docetaxel regime (DOC group, n = 68) or S-l (S-l group, n = 58). The changes of prostate specific antigen (PSA) and alkaline phosphatase (ALP),biochemical and clinical progress time were compared between both groups. Their survival was followed-up and adverse reactions were recorded. The Cox regression proportional risk model was used to analyze the factors that affect the prognosis. Results All 126 cases were evaluable and there were no statistical difference in PSA/ALP declining levels during chemotherapy, immediately after therapy and at 12 months after therapy between both groups. For DOC group and S-l group, the progression time was 9.5 months and 10 months, and the clinical progression time was 11 months and 12.5 months (P>0.05). Among all adverse reactions,the incidence rate of leucocytopenia at grade 3-4 in DOC group was 29.4% (20/68),lower than8.6% of S-l group (5/58). As for gastrointestinal adverse reaction, the incidence rates of diarrhea and abdominal pain were 74.1%(43/58) and 58.6% (34/58) in S-l group, higher than 8.8% (6/68) and 14.7% (10/68) of DOC group (P<0.05). The median survival time of the two groups was 17.0 months (P>0.05). Multifactor analysis showed that the PSA was the only factor affecting the prognosis (HR= 1.654, 95%CI: 1.094-2.500,P=0.017). Conclusion S-l regime got the same treatment effect with DOC regime in treating elderly prostate cancer patients after the failure of androgen deprivation therapy, and the adverse effect were mild. S-l is capable to be considered as an alternative regime for this kind of patients.
