Clinical observation of intrapleural or intraperitoneal hyperthermic perfusion plus bevacizumab on malig-nant pleural or peritoneal effusion

Abstract:Objective To investigate the efficacy of intrapleural or intraperitoneal hyperthermic perfusion plus bevacizumab on malignant pleural or peritoneal effusion. Methods Eighty patients with malignant pleural or peritoneal effusion from June 2011 to March 2013 in our hospital were randomly assigned to receive intrapleural or intraperitoneal hyperthermic perfusion alone ( control group) or intrapleural or intraperitoneal hyperthermic perfusion plus bevacizumab ( observation group) with 40 for each. The regime for hyperthermic perfusion was as follows:cisplatin 30mg, dexamethasone 10mg, fluorouracil 750mg, every 2 weeks for twice. The bevaci-zumab at the dose of 200mg per week were dissolved in 100ml normal saline to inject into intrapleural or intraperitoneal cavity once a week for continuous four weeks. The short-term effect, life quality and adverse reaction were evaluated according to the world health or-ganization ( WHO) standards, quality of life score and the grading standards of antineoplastic adverse reactions. Results All patients successfully completed the treatment. The response rate of observation group was 85?0%, higher than 57?5% of control group with sig-nificant difference ( P<0?05) . However, no significant difference was observed between two groups on disease control rate ( P>0?05) . The quality of life were improved in 16 cases ( 40?0%) , stable in 14 ( 35?0%) and decreased in 10 cases ( 25?0%) of control group, while improved in 34 cases ( 85?0%) , stable in 4 ( 10?0%) and decreased in 2 cases ( 5?0%) of observation group. The observation group?s quality of life was superior to the control group ( P<0?05) . There was no statistically significance between both groups on ad-verse reaction ( P<0?05) . The side effects of two groups mainly was grade 1-2 without serious adverse reaction occured. Conclusion Intrapleural or intraperitoneal hyperthermic perfusion plus bevacizumab on malignant pleural or peritoneal effusion shows better curative effect with few adverse reaction and significantly improved the patients? quality of life.
