Construction of early warning model for hearing loss in elderly patients with refractory hypertension and health management strategies

Abstract:Objective To investigate the influencing factors of hearing loss in elderly patients with refractory hypertension,and to construct a risk warning model,so as to provide guidance for health management centers in taking timely prevention and control measures.Methods A total of 153 elderly patients with refractory hypertension admitted from January 2022 to October 2023 were selected.The demographic characteristics and clinical data of the patients were collected.Multivariate logistic regression model and random forest ensemble classification algorithm were used to analyze the influencing factors of hearing loss in elderly patients with refractory hypertension,and the predictive efficacy of the two was compared.Results Among 153 elderly patients with refractory hypertension,the incidence of hearing loss was 58.17%(89/153);age,diabetes,sleep disorders,serum C reactive protein(CRP),24-hour mean systolic blood pressure(SBP),and 24-hour SBP coefficient of variation were risk factors for hearing loss in elderly patients with refractory hypertension,and the 8-item Morisky Medication Adherence Scale(MMAS-8)score was its protective factor(P<0.01).The factors influencing hearing loss in elderly patients with refractory hypertension,ranked in descending order of importance,were 24-hour mean SBP,MMAS-8 score,24-hour SBP coefficient of variation,serum CRP,diabetes,sleep disorders,and age.The two prediction models had good predictive efficacy in predicting the risk of hearing loss in elderly patients with refractory hypertension,but the performance of the risk warning model constructed by the random forest algorithm was better than that of the logistic regression model,and the area under the curve was 0.914(95%CI:0.858,0.953).Conclusion The random forest prediction model based on age,diabetes,sleep disorders,serum CRP,24-hour mean SBP,24-hour SBP coefficient of variation,and MMAS-8 score has good predictive efficacy and can be applied to clinical practice.It provides a scientific basis for the prevention and control of hearing loss in elderly patients with refractory hypertension.
