Impacts of ultrasound-guided stellate ganglion block combined with general anesthesia on hemodynamics,stress response,and inflammatory factorls in patients undergoing laparoscopic cholecystectomy

Abstract:Objective To investigate the impacts of ultrasound-guided stellate ganglion block(SGB)combined with general anesthesia on hemodynamics,stress response,and inflammatory factor levels in patients undergoing laparoscopic cholecystectomy(LC).Methods Eighty patients admitted from June 2022 to December 2023 for LC were selected,and divided into the control group(n=40)and the observation group(n=40),using a random number table method.The control group received general anesthesia,while the observation group received ultrasound-guided SGB and general anesthesia.The hemodynamic indicators,stress response,inflammatory factor levels,and postoperative pain were compared between the two groups during the perioperative period[upon entering the anesthesia preparation room(T0),at 5 min after SGB(T1),immediately after intubation(T2),immediately after establishing pneumoperitoneum(T3),and immediately after surgery(T4)].Results At T0-T4,both groups showed a trend of first decreasing and then increasing in heart rate and mean arterial pressure(MAP)(P<0.01),and at T1-T4,the heart rate and MAP of the observation group were more stable than those of the control group(P<0.05,P<0.01).At T2-T4,the levels of cortisol and adrenocorticotropic hormone(ACTH)in both groups increased compared with those at T0,and the increase in the observation group was smaller than that in the control group(P<0.05,P<0.01).At T2-T4,the levels of C reactive protein(CRP)and tumor necrosis factor-α(TNF-α)in both groups increased compared with those at T0,which were lower in the observation group than in the control group(P<0.01).Immediately after anesthesia recovery,and at 12,24,48,and 72 after surgery,visual analog scale(VAS)pain scores in both groups showed a trend of initial increase and then decrease(P<0.01),and the VAS scores of the observation group were lower than those of the control group(P<0.01).Conclusion Ultrasound-guided SGB combined with general anesthesia can effectively alleviate stress response,reduce hemodynamic fluctuations,and lower levels of inflammatory factors in patients undergoing LC.
