Diagnostic value of aldosterone-to-renin ratio,calcium-phosphorus product,and serum potassium in primary aldosteronism

Abstract：Objective To investigate the diagnostic value of aldosterone-to-renin ratio(ARR),calcium-phosphorus product,and serum potassium for primary aldosteronism(PA).Methods A total of 93 PA patients treated between April 2024 and March 2025 were selected as the PA group,and 58 patients with essential hypertension(EH)during the same period were selected as the EH group.Clinical data,ARR,calcium-phosphorus product,and serum potassium levels were collected from both groups for comparison.Multivariate logistic regression analysis was performed to identify influencing factors for PA development in hypertensive patients.Receiver operating characteristic(ROC)curves were used to evaluate the diagnostic value of ARR,calcium-phosphorus product,and serum potassium levels for PA.Results Compared with the EH group,the PA group exhibited a longer duration of hypertension,higher diastolic blood pressure(DBP),systolic blood pressure(SBP),plasma renin activity,aldosterone levels,and ARR,as well as lower calcium-phosphorus product and serum potassium levels(P＜0.05,P＜0.01).Multivariate logistic regression analysis revealed that the duration of hypertension,DBP,SBP,serum potassium,ARR,and calcium-phosphorus product were all influencing factors for PA development in hypertensive patients(P＜0.05,P＜0.01).ROC curve analysis demonstrated that the areas under the curve for diagnosing PA were 0.805 for ARR,0.750 for calcium-phosphorus product,and 0.785 for serum potassium,with a combined diagnostic AUC of 0.900.Conclusion The duration of hypertension,DBP,SBP,serum potassium,ARR,and calcium-phosphorus product are all influencing factors for the occurrence of PA in hypertensive patients.ARR,calcium-phosphorus product,and serum potassium levels have certain diagnostic value for PA,and their combined diagnostic efficacy is higher.
