Anesthetic efficacy of Remimazolam combined with Alfentanil for percutaneous tracheotomy

Abstract:Objective To investigate the anesthetic efficacy of Remimazolam combined with Alfentanil for percutaneous tracheotomy.Methods A total of 106 patients undergoing percutaneous tracheotomy from August 2021 to July 2024 were selected and divided into group A(n=53,receiving anesthesia with Remimazolam combined with Alfentanil)and group B(n=53,receiving anesthesia with Propofol combined with Alfentanil)by random number table method.The hemodynamic indicators[systolic blood pressure(SBP),heart rate(HR),diastolic blood pressure(DBP),and pulse oxygen saturation(SpO2)],stress indicators[cortisol(Cor),norepinephrine(NE)],Obeserver's Assessment of Alertness/Sedation(OAA/S)score,bispedic index(BIS)at different time points[before the operation(T1),after the injection of anesthetic drugs(T2),at the time of tracheotomy and cannulated tube insertion(T3),and at the end of the operation(T4)],surgery-related indicators(duration of operation,frequency of airway response,time to recovery),and the occurrence of perioperative adverse reactions were compared between the two groups.Results SBP,HR and DBP in group A were higher than those in group B at T2,which,however,were lower in group A than in groups B at T3 and T4(P<0.05),but there was no significant difference in SpO2 between the two groups at T1-T4(P>0.05).At T2,T3 and T4,Cor and NE in group A were lower than those in group B(P<0.05).The OAA/S scores of group A at T3 and T4 were lower than those of group B,and the BIS values at T2,T3 and T4 were lower than those of group B(P<0.01).There was no significant difference in the duration of operation and time to recovery between the two groups(P>0.05),and the number of airway responses in group A was fewer than that in group B(P<0.01).There was no significant difference in the total incidence of perioperative adverse reactions between the two groups(P>0.05).Conclusion Anesthesia with Remimazolam combined with Alfentanil is superior to that with Propofol combined with Alfentanil in ensuring the sedative effect and reducing stress responses in patients undergoing percutaneous tracheotomy.Moreover,it can effectively reduce hemodynamic fluctuations,decrease the frequency of airway responses,and effectively ensure anesthesia safety.
