Effect of the modified segmental ligation technique with abdominal thin skin flap in repairing multiple-digit degloving injuries and its impact on hand function

Abstract:Objective To analyze the efficacy of the modified segmental ligation technique with abdominal thin skin flap in repairing multiple-digit degloving injuries and its impact on hand function.Methods A total of 118 patients with multi-finger skin degloving injury admitted from January 2022 to May 2024 were selected and divided into modified group(n=60)and control group(n=58)according to different treatment schemes.The modified group was treated with modified segmental ligation technique with abdominal thin skin flap,while the control group was treated with pedicled ilioinguinal flap.The therapeutic effect,time to flap pedicle division,cumulative hospitalization duration,blood perfusion of the flaps at 1 d after flap repair and at 7,15,30 d after pedicle division were compared between the two groups.The range of motion(ROM)of metacarpophalangeal joint and proximal interphalangeal joint of the affected hand at 1 month,6 months and 1 year after operation,the British Medical Research Council(BMRC)sensory recovery grading,and patient satisfaction at 1 year after operation were compared between the two groups.Results The excellent and good rate of treatment in both groups was 100.00%,and there was no significant difference between the two groups(P>0.05).The time to pedicle division and cumulative hospitalization duration in the modified group were shorter than those in the control group,showing significant differences(P<0.01).The capillary filling time of the flap in the modified group was shorter than that in the control group at 7,15 and 30 d after pedicle division,and the blood perfusion of the flap was better than that in the control group at 7,15 and 30 d after operation(P<0.01).The ROM of metacarpophalangeal joint and proximal interphalangeal joint in the affected hand/fingers in the two groups increased gradually at 1 month,6 months and 1 year after operation,which was better in the modified group than in the control group(P<0.05).At 6 months and 1 year after operation,the improvement of BMRC sensory recovery grading in the modified group was better than that in the control group(P<0.05,P<0.01).The satisfaction of patients in the modified group was higher than that in the control group at 1 year after operation(P<0.05).Conclusion The modified segmental ligation technique with abdominal thin skin flap for repairing multiple-digit skin degloving injury enables pedicle division in advance,promote the recovery of skin flap blood supply,ameliorates sensory function and ROM of the hand,and improve patient satisfaction.
