Value of OMERACT semi-quantitative ultrasound scoring system in longitudinal assessment of the efficacy of gouty arthritis treatment

Abstract:Objective To investigate the value of the semi-quantitative ultrasound scoring system(SQUS)developed by the Outcome Measures in Rheumatology Clinical Trials(OMERACT)Working Group in evaluating the efficacy of gouty arthritis(GA)treatment.Methods A total of 94 patients with GA treated from January 2023 to June 2024 were selected.Before treatment,they were divided into the mild group(n=46)and the moderate-to-severe group(n=48)based on the 28-joint disease activity score(DAS28).The general data and SQUS scores of the two groups were compared.At 6 and 12 months after standardized treatment,the levels of blood biochemical parameters and SQUS scores,including dual-contour sign(DC),tophi,aggregate score and composite score of DC+tophiscore+aggregate(DTA),of all patients before and after treatment were observed and compared,and the correlation between DTA score and blood biochemical parameters at different time points was analyzed.Results Before treatment,there were significant differences in the frequency of gout attacks,blood uric acid(BUA),and C reactive protein(CRP)levels between the two groups(P<0.01).Before treatment,the SQUS scores,including DC,tophi,aggregate score and DTA score of patients in the mild group were significantly lower than those in the moderate-to-severe group(P<0.01).After treatment,the levels of BUA and CRP of patients decreased significantly,and the SQUS scores,including DC,tophi,aggregate score and DTA score all decreased significantly,tophi,aggregate score and DTA score all decreased significantly(P<0.05).The SQUS DTA scores of GA patients before treatment,and at 6 and 12 months after treatment were positively correlated with the levels of BUA and CRP(P<0.05).Conclusion The SQUS score can effectively reflect the disease activity of GA patients.DC,tophi and aggregate scores can also be used as important reference bases for evaluating the therapeutic effect of GA patients.
