Clinical efficacy of a modified technique using balloon dilation catheter-assisted peripheral thrombus aspiration catheters for lower extremity arterial thrombotic diseases

Abstract:Objective To analyze the clinical effect of the modified technique using balloon dilation catheter-assisted peripheral thrombus aspiration catheters in the treatment of lower extremity arterial thrombotic disease.Methods A retrospective analysis was conducted on the clinical data of 15 patients with lower extremity arterial thrombotic disease who were treated with a modified technique using balloon dilation catheter-assisted peripheral thrombus aspiration catheters from January to December 2024.The severity of lower extremity arterial ischemia of the patients was evaluated by the Rutherford classification before and after the operation,and blood perfusion in the ischemic limbs was evaluated by the thrombolysis in myocardial infarction(TIMI)blood flow grade,based on which the clinical efficacy was evaluated.Results The surgical success rate of the modified technique using balloon dilation catheter-assisted peripheral thrombus aspiration catheters in 15 patients with lower extremity arterial thrombotic disease was 100.0%in all patients,and the thrombectomy success rate was 80.0%(12/15).According to TIMI flow grade in lower extremity arteries,there were 11 cases of grade 0 and 4 cases of grade 1 before the operation,as well as 3 cases of grade 1 and 12 cases of grade 2 immediately after the initiation of operation.The Rutherford classification assessment of the severity of ischemia in patients showed that there were 7 cases of grade ⅡA and 8 cases of grade ⅡB before the operation,as well as 10 cases of grade 0 and 5 cases of grade I after the operation.Three cases(20.0%)had poor postoperative improvement.They were given catheter contact thrombolytic therapy for 21.4 to 48.0 h,and later their TIMI flow grade was grade 1.During the perioperative period,none of the 15 patients experienced massive hemorrhage,acute renal insufficiency,or severe cardiovascular and cerebrovascular complications.After a 3-month follow-up after discharge,the TIMI flow grade was as follows:11 cases were grade 2 and 4 cases were grade 1.Based on Rutherford classification,8 cases were grade 0 and 7 cases were grade I.There were no amputations or deaths,and the limb salvage rate was 100.0%(15/15).Conclusion The modified technique using balloon dilation catheter-assisted peripheral thrombus aspiration catheters for patients with lower extremity arterial thrombotic disease has the characteristics of minimal surgical trauma,few postoperative complications,high limb salvage rate,simple operation and definite therapeutic effect.It is a safe and effective short-term therapeutic option for patients with acute onset of lower extremity arterial thrombotic disease.
