Effect of ultrasound and iontophoresis-mediated transdermal delivery of Chinese herbal medicine combined with Yiqi Huoxue Tongbi formula on nerve conduction velocity and islet function in patients with diabetic peripheral neuropathy

Abstract:Objective To explore the clinical effect of ultrasound and iontophoresis-mediated transdermal delivery of Chinese herbal medicine combined with Yiqi Huoxue Tongbi formula in the treatment of patients with diabetic peripheral neuropathy(DPN).Methods A total of 102 patients with DPN who visited the hospital from February 2023 to March 2024 were selected as the research subjects and divided into the control group(n=51)and the observation group(n=51)according to the random number table method.Both groups received standardized treatment.The control group was supplemented with the Yiqi Huoxue Tongbi formula,while the observation group was treated with ultrasound and iontophoresis-mediated transdermal delivery of Chinese herbal medicine on the basis of the control group.The therapeutic effects and safety of the two groups were compared,as well as the Toronto Clinical Scoring System(TCSS)score,homeostasis model assessment of insulin resistance(HOMA-IR),homeostasis model assessment of β-cell function(HOMA-β),interleukin-6(IL-6),soluble intercellular adhesion molecule-1(sICAM-1),endothelin-1(ET-1),and fasting C-peptide(C-P),nitric oxide(NO),tumor necrosis factor-α(TNF-α),fasting blood glucose(FBG),and the levels of sensory nerve conduction velocity(SNCV)and motor nerve conduction velocity(MNCV)of the common peroneal nerve and median nerve.Results The total effective rate of the observation group was 90.20%(46/51),which was higher than that of the control group[74.51%(38/51)](P<0.05).After treatment,the symptoms,sensory test,reflex score and total TCSS score of the observation group were all lower than those of the control group(P<0.05).After treatment,HOMA-IR,sICAM-1,IL-6,ET-1,TNF-α and FBG in the observation group were lower than those in the control group,while C-P,NO,HOMA-[3,as well as MNCV and SNCV of the median nerve and common peroneal nerve were higher than those in the control group(P<0.05).There was no statistical difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Ultrasound and iontophoresis-mediated transdermal delivery of Chinese herbal medicine combined with Yiqi Huoxue Tongbi formula is reliable and safe in the treatment of DPN patients.It can effectively alleviate clinical symptoms and inflammatory damage,regulate insulin metabolism and restore nerve conduction velocity.
