Analysis of the characteristics of treatment adherence to anti-TNF-α biologics and potential influencing factors in patients with moderate-to-severe plaque psoriasis

Abstract:Objective To analyze the treatment adherence to anti-tumor necrosis factor-α(TNF-α)biologics in patients with moderate-to-severe plaque psoriasis,and to identify potential related factors that may affect patient adherence.Methods A total of 222 patients with moderate-to-severe plaque psoriasis receiving combined anti-TNF-α biologics from June 2022 to June 2024 were selected as the research subjects.According to the Morisky Medication Adherence Scale(MMAS-8)score,the patients were divided into the good adherence group(MMSA-8 score ≥6 points)and the poor adherence group(MMSA-8 score<6 points).The differences in sociodemographic data,treatment plans and treatment experiences were compared between the two groups.Multivariate logistic regression analysis was used to screen the influencing factors of poor adherence in patients with moderate-to-severe plaque psoriasis treated with anti-TNF-α biologics.Results A total of 222 valid adherence questionnaires were retrieved(with a recovery rate of 100%),and the poor adherence rate of the enrolled patients was 41.44%(92/222).The proportion of patients with a college degree or above,the total score of the Social Support Rating Scale(SSRS),and the total score of the Knowledge,Attitude and Practice(KAP)Questionnaire in the poor adherence group were all lower than those in the good adherence group,while the proportion of patients with ≥3 types of combined topical medications and low long-term safety of drug application was higher than that in the good adherence group(P<0.05).Multivariate logistic regression analysis indicated that the total score of SSRS,the total score of the KAP Questionnaire,educational level,types of combined topical medications,and the long-term safety of drug use were all important factors affecting the adherence of patients with moderate-to-severe plaque psoriasis treated with anti-TNF-α biologics(P<0.05).Conclusion The adherence of patients with moderate-to-severe plaque psoriasis in the treatment with TNF-α biologics still needs to be improved.Moreover,patients who have a relatively low score of total SSRS and the KAP Questionnaire,a relatively low educational level,a wide variety of combined topical medications,and poor long-term safety of drug use have poor adherence after receiving anti-TNF-α biologics.
