Clinical analysis of ultrasonic misdiagnosis of scrotal leiomyoma

Abstract:Objective To summarize the causes of ultrasonic misdiagnosis of scrotal leiomyoma and to propose preventive strategies,so as to enhance the recognition of sonographers and reduce the preoperative misdiagnosis rate.Methods A retrospective analysis was conducted on the medical records of 10 patients with scrotal leiomyoma misdiagnosed who were admitted from February 2017 to February 2023.Results All 10 patients had no obvious clinical symptoms.They visited the hospital by accidental palpation of scrotal masses or the discovery of lesions protruding from the scrotal wall.The interval from the initial discovery of scrotal masses to the hospital visit was 2 months to 4 years.All laboratory tests showed no obvious abnormalities and the tumor markers were not high.Laterality distribution of the 10 scrotal lesions was as follow:3 were bilateral,5 were on the left side,and 2 were on the right side.Six cases of lesions were located within the scrotum,and four cases protruded from the surface of the scrotum.Ultrasound examination revealed heterogeneous masses with clear envelopes and well-defined boundaries.The lesions were mainly solid,and the internal echoes varied in strength and were rather disordered.The mass had a distinct boundary with the testis,epididymis and spermatic cord.Color Doppler flow imaging showed a relatively rich blood flow signal within the lesion.Four cases of testicular tumors,two cases of dermoid cysts or epidermoid cysts,two cases of fibromas,and two cases of lipomas were considered.They underwent elective surgery,and all were confirmed as scrotal leiomyomas by postoperative pathological examination.The misdiagnosis lasted 3 to 5 d.The patient was followed up for 2 years after the operation,and no symptoms or recurrence was reported.Conclusion When an ultrasound examination of a patient reveals an abnormal echo mass within the scrotum or protruding from the scrotal wall,which has a clear boundary with the testicles,epididymis and spermatic cord,and the lesion has rich blood flow,the possibility of scrotal leiomyoma should be considered.Pathological examination can be performed as early as possible to confirm the diagnosis.
