Application of the theory of simultaneous treatment of bone and soft tissue in postoperative rehabilitation after wrist arthroscopy-assisted management of Herbert type B scaphoid fracture combined with triangular fibrocartilage complex injury

Abstract:Objective To investigate the application value of minimally invasive wrist arthroscopy guided by the theory of"simultaneous treatment of bone and soft tissue"in postoperative rehabilitation after the management of Herbert type B scaphoid fractures combined with triangular fibrocartilage complex(TFCC)injury.Methods A total of 86 patients with Herbert type B scaphoid fractures and TFCC injury admitted between January 2021 and October 2023 were prospectively selected.According to the random number table method,patients were divided into the observation group and the control group,with 43 patients in each group.The observation group underwent arthroscopically-assisted scaphoid fracture fixation and TFCC injury repair,followed by postoperative rehabilitation training combined with external herbal fumigation and washing.The control group received arthroscopically-assisted scaphoid fracture fixation and TFCC injury repair,followed by postoperative rehabilitation training alone.Visual Analog Scale(VAS)pain score,grip strength,wrist flexion-extension range of motion(ROM),forearm pronation-supination ROM,and Cooney wrist score were compared between the two groups at 3,6,and 12 months postoperatively.Results At 3 months postoperatively,the observation group showed greater wrist flexion-extension ROM and forearm pronation-supination ROM than the control group(P<0.05).Grip strength in the observation group was greater than that in the control group at 3 and 6 months postoperatively(P<0.05).At 3 months postoperatively,the VAS score in the observation group was lower than that in the control group(P<0.05),and at 3 and 6 months postoperatively,the Cooney wrist score in the observation group was higher than that in the control group(P<0.05,P<0.01).Conclusion The combination of the"simultaneous treatment of bone and soft tissue"theory and wrist arthroscopy for Herbert type B scaphoid fractures with TFCC injury can significantly improve ROM of wrist joint and functional scores.
