Causes of misdiagnosis and ultrasonic characteristics of ovarian corpus luteum rupture with acute lower abdominal pain as the initial symptom

Abstract:Objective To analyze the causes of misdiagnosis of ovarian corpus luteum rupture with acute lower abdominal pain as the initial symptom,and to explore its ultrasonic characteristics and countermeasures to reduce misdiagnosis.Methods A retrospective analysis was conducted on the clinical data of 6 patients with ovarian corpus luteum rupture misdiagnosed as acute appendicitis from May 2021 to May 2023.Results All 6 patients presented mainly with sudden acute lower abdominal pain,and 5 of them were accompanied by gastrointestinal symptoms such as nausea and vomiting.Physical examination showed tenderness and rebound tenderness in the right or left lower quadrant,and the initial diagnosis was acute appendicitis.Among them,the symptoms of 4 patients did not improve after receiving anti-infection treatment,and 2 patients were scheduled to undergo appendectomy.Due to poor therapeutic effect or progressive decrease in hemoglobin found in preoperative assessment,further examinations were conducted.Ultrasound examination revealed pelvic effusion and abnormal ovarian echoes,and the diagnosis was ovarian corpus luteum rupture.The misdiagnosis lasted 1 to 3 d.After diagnosis,5 patients recovered and were discharged after 5 to 9 d of conservative treatment,and there was no recurrence during the 3-month follow-up.One patient underwent emergency laparoscopic surgery for hemostasis due to a large amount of intra-abdominal bleeding.The patient was discharged at 5 d after the operation and recovered well after a 6-month follow-up.Conclusion Ovarian corpus luteum rupture has similar clinical manifestations to acute appendicitis and is prone to misdiagnosis.Ultrasound examination can show abnormal echoes in the adnexal area and pelvic effusion,and has important diagnostic value.Clinicians should enhance their understanding of ovarian corpus luteum rupture.A careful differential diagnosis is necessary for women of childbearing age with acute lower abdominal pain,and timely ultrasound examinations should be performed to reduce misdiagnosis.
