Effect of modified Zhigancao decoction in the auxiliary treatment of postural tachycardia syndrome with deficiency of both qi and yin and its impact on cardiac function,electrocardiogram parameters and RAAS

Abstract：Objective To investigate the effect of modified Zhigancao decoction in the auxiliary treatment of postural tachycardia syndrome with deficiency of both qi and yin and its impact on cardiac function,electrocardiogram parameters and renin-angiotensin-aldosterone system(RAAS).Methods A total of 80 patients with postural tachycardia syndrome with deficiency of both qi and yin admitted from June 2022 to June 2024 were selected and divided into the observation group(n=40)and the control group(n=40)according to the random number table method.The control group was treated with Bisoprolol,and the observation group was treated with modified Zhigancao decoction on the basis of the control group.Both groups were treated continuously for 4 weeks.The therapeutic effects of the two groups were compared,as well as the cardiac function indexes[stroke volume(SV),cardiac output(CO)],electrocardiogram parameter QT dispersion(QTd),heart rate variability(HRV)parameters[standard diviation of NN RR intervals(SDNN),standard deviation of the average NN RR intervals(SDANN),root mean square of successive NN RR differences(RMSSD),percentage of successive NN RR intervals that differ by more than 50 milliseconds(PNN50)],RAAS indexes[renin,angiotensin Ⅱ(Ang Ⅱ),aldosterone(ALD)],quality of life scores before and after treatment and adverse reactions.Results After treatment,the total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,SV and CO in the two groups were higher than those before treatment,which were higher in the observation group than in the control group(P<0.05,P<0.01).After treatment,the QTd of the two groups was lower than that before treatment,which was lower in the observation group than in the control group(P<0.05,P<0.01).After treatment,SDNN,SDANN,RMSSD and PNN50 in the two groups were higher than those before treatment,and higher in the observation group than in the control group(P<0.05,P<0.01).After treatment,the levels of serum renin,AngⅡ and ALD in the two groups were lower than those before treatment,and lower in the observation group than in the control group(P<0.05,P<0.01).After treatment,the scores of mental function,physical function,social function and material life of 36-item short form health survey in the two groups were higher than those before treatment,and the scores in the observation group were higher than those in the control group(P<0.05,P<0.01).There was no significant difference in the total incidence of adverse reactions between the two groups during treatment(P>0.05).Conclusion The effect of modified Zhigancao decoction combined with Bisoprolol in the treatment of patients with postural tachycardia syndrome with deficiency of both qi and yin is definite.It can improve the cardiac function,electrocardiogram parameters and HRV parameters of patients,inhibit the excessive activation of RAAS system,improve the quality of life,and has certain safety.
