Analysis of the effect and prognosis of HRCT-guided radiofrequency ablation in the treatment of patients with early-stage non-small cell lung cancer

Abstract:Objective To explore the effect of radiofrequency ablation(RFA)guided by high-resolution computed tomography(HRCT)in the treatment of patients with early-stage non-small cell lung cancer(NSCLC)and to analyze its prognosis.Methods Eighty patients with early-stage NSCLC diagnosed and treated from May 2022 to September 2023 were selected.Due to poor cardiopulmonary function or other concurrent diseases,they were unable to tolerate surgery or declined surgical treatment,and then received HRCT-guided RFA.After a 6-month follow-up,the therapeutic effect and prognosis were evaluated,and they were divided into the good prognosis group and the poor prognosis group according to the prognosis.Multivariate logistic regression analysis was performed to analyze the risk factors for poor prognosis of HRCT-guided RFA in patients with early-stage NSCLC were analyzed.Results Among 80 patients with early-stage NSCLC,there were 80 lesions,including 61 solid lesions and 19 subsolid lesions.The lesions were located in the right upper lung in 22 patients,the right middle lung in 14 patients,the right lower lung in 18 patients,the left upper lung in 14 patients,and the left lower lung in 12 patients.The maximum diameters of the lesions in the patients gradually decreased from 1 d to 6 months after the operation(P<0.05).Among the 80 patients with early-stage NSCLC,64 had a good prognosis at 6 months after treatment(including 21 patients with complete remission and 43 with partial remission),and 16 had a poor prognosis(including 14 patients experiencing recurrence and 2 deaths after the operation).During the treatment process,28 complications occurred,including 10 cases(35.71%)of hemoptysis,8 cases(28.57%)of pneumothorax,7 cases(25.00%)of pneumonia,and 3 cases(10.71%)of pleural effusion.The results of multivariate logistic regression analysis showed that tumor type squamous cell carcinoma and tumor maximum diameter>2 cm were risk factors for poor prognosis of HRCT-guided RFA treatment in patients with early-stage NSCLC(P<0.05,P<0.01).Conclusion HRCT-guided RFA has certain effects in the treatment of early-stage NSCLC patients,with relatively low incidence of complications during the treatment process.Tumor type squamous cell carcinoma and tumor maximum diameter>2 cm are risk factors for poor prognosis of HRCT-guided RFA in early-stage NSCLC patients.
