Correlation and diagnostic value of amplitude integrated electroencephalogram score,blood lactic acid and neonatal beh avioral neurological assessment score of late preterm infants with brain injury

Abstract:Objective To investigate the correlation between amplitude integrated electroencephalogram(aEEG)score,blood lactic acid and neonatal behavioral neurological assessment(NBNA)score in late preterm infants with brain injury,and to analyze the diagnostic value.Methods A total of 215 late preterm infants admitted from September 2019 to August 2021 were selected and divided into brain injury group and non-brain injury group according to the presence or absence of brain injury.The clinical data of the two groups were compared.The aEEG scores and blood lactic acid levels of the two groups were detected and evaluated for the first time and second time,and the NBNA scores of the two groups were evaluated for the first time and second time.Pearson correlation coefficient method was used to analyze the correlation between aEEG score,blood lactic acid level and NBNA score.Multivariate logistic regression analysis was used to analyze the influencing factors of brain injury in late preterm infants.The receiver operating characteristic(ROC)curve was drawn to analyze the value of aEEG score and blood lactic acid in the diagnosis of brain injury in late preterm infants.Results The aEEG continuity score,sleep-wake cycle score and total score detected and evaluated for the first and second time in brain injury group were lower than those in the non-brain injury group,while the blood lactic acid detected for the first and second time was higher than that in the non-brain injury group,showing significant differences(P<0.05,P<0.01).The NBNA scores evaluated for the first and second time brain injury group were lower than those of the non-brain injury group,showing significant differences(P<0.01).Pearson correlation coefficient analysis showed that aEEG continuity score,sleep-wake cycle score and aEEG total score detected and evaluated for the first and second time in late preterm infants were positively correlated with NBNA score,while blood lactic acid level was negatively correlated with NBNA score(P<0.01).Multivariate logistic regression analysis showed that the first detection of aEEG continuity score,sleep-wake cycle score,total score and blood lactic acid detected and evaluated for the first time were independent influencing factors of brain injury in late preterm infants(P<0.01).ROC curve analysis showed that the area under the curve of aEEG continuity score,sleep-wake cycle score,total score and blood lactic acid in the diagnosis of brain injury in late preterm infants was 0.912(95%CI:0.865,0.946),the sensitivity was 82.93%,and the specificity was 86.47%,which was higher than that of each index alone(P<0.01).Conclusion aEEG continuity score,sleep-wake cycle score,total score detected within 72 h after birth and blood lactic acid in the first detection(within 1 h after birth)are independent influencing factors of brain injury in late preterm infants,which have a significant correlation with NBNA score.The combined detection of aEEG score and blood lactic acid has a high diagnostic value for brain injury in late preterm infants.
