Value of synthetic magnetic resonance quantitative parameters combined with serum miR-142-3p detection in the evaluation of the degree of lumbar intervertebral disc degeneration and the prediction of surgical efficacy in patients with lumbar disc herniation

Abstract:Objective To explore the value of synthetic magnetic resonance quantitative parameters combined with serum miR-142-3p detection in the evaluation of the degree of lumbar intervertebral disc degeneration and the prediction of surgical efficacy in lumbar disc herniation(LDH).Methods A total of 150 patients with LDH who underwent surgical treatment from January 2020 to December 2023 were selected as the research subjects.Magnetic resonance synthetic images were analyzed before the operation,and the expression level of serum miR-142-3p was detected.The patients were followed up for one year after the operation and the surgical efficacy was evaluated according to the modified Macnab standard:"excellent"and"good"were defined as effective(effective group)"fair"and"poor"were defined as ineffective(ineffective group).The baseline data,quantitative parameters of synthetic magnetic resonance,and the expression level of serum miR-142-3p were compared between the two groups.The quantitative parameters of synthetic magnetic resonance and the expression level of serum miR-142-3p in patients with different Pfirrmann grades were also compared.Spearman analysis was used to analyze the correlations of quantitative parameters of synthetic magnetic resonance,and serum miR-142-3p expression with Pfirrmann grade and surgical efficacy of patients.Multivariate Logistic regression analysis was used to analyze the relationships between quantitative magnetic resonance parameters,serum miR-142-3p expression and surgical efficacy of patients.The receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of quantitative parameters of synthetic magnetic resonance and serum miR-142-3p expression for surgical efficacy.Results The proportion of Pfirrmann grade Ⅲ,the T1 values,T2 values,PD values level of the nucleus pulposus and annulus fibrosus,and the expression of serum miR-142-3p in the ineffective group were all lower than those in the effective group,showing significant differences(P<0.01).The T1 values,T2 values,PD values of the nucleus pulposus and annulus fibrosus and the expression level of serum miR-142-3p in patients with Pfirrmann grade Ⅲ were all higher than those in patients with Pfirrmann grade Ⅳ,showing significant difference(P<0.05,P<0.01).Spearman correlation analysis showed that the T1,T2,PD values of the nucleus pulposus and annulus fibrosus,as well as the expression level of serum miR-142-3p,were negatively correlated with the degree of lumbar intervertebral disc degeneration and surgical efficacy in patients with LDH(P<0.01).Multivariate Logistic regression analysis showed that the quantitative parameters of synthetic magnetic resonance,the T1 value,T2 value,PD value of the nucleus pulposus and annulus fibrosus,and the expression level of serum miR-142-3p,were the influencing factors of the surgical efficacy in patients with LDH(P<0.01).ROC curve analysis showed that the area under the curve(AUC)of the quantitative parameters of synthetic magnetic resonance,the T1 value,T2 value,and PD value of the nucleus pulposus and annulus fibrosus combined with serum miR-142-3p,for predicting the surgical efficacy of LDH patients was the largest,which was significantly greater than that predicted by each index alone.Conclusion Quantitative parameters of synthetic magnetic resonance and serum miR-142-3p are closely related to the degree of lumbar intervertebral disc degeneration and surgical efficacy in patients with LDH.Moreover,the combined detection of quantitative parameters of synthetic magnetic resonance and serum miR-142-3p has a relatively high value in evaluating the surgical efficacy of patients with LDH.
