Efficacy and safety of Dapagliflozin combined with Metformin sustained-release tablets in the treatment of elderly type 2 diabetes mellitus with high risk of hypoglycemia and cardiovascular diseases

Abstract:Objective To analyze the efficacy and safety of Dapagliflozin combined with Metformin sustained-release tablets in the treatment of elderly type 2 diabetes mellitus(T2DM)with high risk of hypoglycemia and cardiovascular disease.Methods A total of 1 488 elderly T2DM patients with high risk of hypoglycemia and cardiovascular disease admitted from January 2021 to June 2024 were selected and divided into group M(Metformin sustained-release tablets,n=496),group D(Dapagliflozin,n=496)and group DM(Dapagliflozin combined with Metformin sustained-release tablets,n=496)by random number table method.Blood glucose,blood pressure,blood lipid,cardiac function and safety were compared between the three groups before and after treatment.Results At 1 and 3 months after treatment,fasting blood glucose,glycosylated hemoglobin,triglyceride,low-density lipoprotein cholesterol,and total cholesterol in group DM were lower than those in groups D and M,while high-density lipoprotein cholesterol was higher than that in groups D and M(P<0.05),and there was no significant difference between group D and group M(P>0.05).At 1 and 3 months after treatment,the systolic blood pressure and diastolic blood pressure were lower in group DM than in group D and group M,and lower in in group D than in group M(P<0.05).At 3 months after treatment,the left ventricular end-diastolic diameter and B-type natriuretic peptide in group DM were lower than those in group D and group M,while the left ventricular ejection fraction was higher than that in group D and group M,and higher in group D than in group M(P<0.05).The total incidence of adverse reactions in group D was 0.20%(1/496),which was lower than 1.81%(9/496)in group DM and 2.42%(12/496)in group M(P<0.05),and there was no significant difference between group DM and group M(P>0.05).Conclusion Dapagliflozin combined with Metformin sustained-release tablets is effective in the treatment of elderly T2DM patients with high risk of hypoglycemia and cardiovascular disease,which can effectively reduce the levels of blood glucose,blood pressure and blood lipids,improve heart function,and has relatively good safety.
