Changes in serum sCD44v6 and Cyfra21-1 levels in patients with middle and advanced esophageal cancer and their clinical significance

Abstract:Objective To analyze the changes and clinical significance of soluble CD44 variant 6(sCD44v6)and cytokeratin 19 fragment antigen 21-1(Cyfra21-1)levels in serum of patients with middle and advanced esophageal cancer.Methods In total,80 patients with middle and advanced esophageal cancer admitted from March 2020 to March 2023 were retrospectively selected and included in the research group,and 50 healthy people who underwent physical examination during the same period were selected as the control group.The general data of patients with middle and advanced esophageal cancer were collected and sorted out.Serum levels of sCD44v6 and Cyfra21-1 were compared between the two groups.After 12 to 18 months of follow-up,patients were categorized into the survival subgroup(n=57)and the death subgroup(n=23)based on their survival status,and serum levels of sCD44v6 and Cyfra21-1 were compared.In addition,Cox regression model was used to analyze the risk factors affecting the survival of patients.Results The levels of serum sCD44v6 and Cyfra21-1 in the research group were higher than those in the control group(P<0.01).Serum sCD44v6 and Cyfra21-1 levels in patients with middle and advanced esophageal cancer with tumor stage Ⅳ,moderate to low differentiation degree and lymph node metastasis were higher than those in patients with TNM stage Ⅲ,high differentiation degree and no lymph node metastasis(P<0.01).The levels of serum sCD44v6 and Cyfra21-1 in the death subgroup were higher than those in the survival subgroup(P<0.05,P<0.01).High levels of sCD44v6 and Cyfra21-1 were risk factors affecting the survival of patients with middle and advanced esophageal cancer(P<0.05).By the end of follow-up,the survival rate of patients with middle and advanced esophageal cancer and high levels of serum sCD44v6 and Cyfra21-1 was lower than that of patients with low levels(P<0.05).Conclusion Serum sCD44v6 and Cyfra21-1 are abnormally elevated in patients with middle and advanced esophageal cancer,and the two levels are related to tumor staging,differentiation degree and lymph node metastasis.The detection of serum sCD44v6 and Cyfra21-1 has certain significance for the auxiliary diagnosis and prognosis evaluation of esophageal cancer.
