Clinical effect of Dangshen Pingfei Huoxue decoction in the treatment of pulmonary fibrosis with qi-yin deficiency and blood stasis syndrome and its effect on TGF-β1/Smad signaling pathway

Abstract：Objective To explore the clinical effect of Dangshen Pingfei Huoxue decoction in the treatment of pulmonary fibrosis with qi-yin deficiency and blood stasis syndrome and its impact on the transforming growth factor β1 protein(TGF-β1)/Smad protein(Smad)signaling pathway.Methods A total of 122 patients with pulmonary fibrosis syndrome of qi-yin deficiency with blood stasis who were admitted from June 2021 to June 2024 were selected.According to the random number table method,they were divided into the observation group and the control group,with 61 patients in each group.The control group was given the conventional Western medical treatment,while the observation group was supplemented with Dangshen Pingfei Huoxue decoction on the basis of the conventional Western medical treatment.Both groups were treated for 2 months.The clinical effects and adverse reactions during the treatment period were compared between the two groups.Before treatment and at 2 months after treatment,6 min walking distance(6MWD),arterial oxygen saturation(SaO2),forced expiratory volume in one second(FEV1)/forced vital capacity(FVC),high-resolution CT(HRCT)score,total TCM syndrome score,Borg scale score,and expression levels of TGF-β1/Smad signaling pathway proteins[transforming growth factor-β1(TGF-β1),Mothers against decapentaplegic homolog 3(Smad 3),Smad 7],and pulmonary fibrosis indicators[prohyaluronic acid(HA),type Ⅲ procollagen peptide(PⅢNP),type I collagen peptide(PⅠCP)]were compared.Results The total effective rate of the observation group[96.72%(59/61)]was higher than that of the control group[85.25%(52/61)](P<0.05).At 2 months after treatment,the 6MWD,SaO2,and FEV1/FVC in the observation group were higher than those in the control group,while the TCM syndrome score,HRCT score,Borg score,and the levels of TGF-β1,Smad 3,Smad 7,PⅠCP,PⅢNP,and HA were lower than those in the control group(P<0.05,P<0.01).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The treatment of pulmonary fibrosis with qi-yin deficiency and blood stasis syndrome using combined Dangshen Pingfei Huoxue decoction has a remarkable effect.It can effectively improve pulmonary function,relieve dyspnea,delay the progression of pulmonary fibrosis,and facilitate the prognosis of the disease.
