Efficacy and safety analysis of Osimertinib combined with platinum-based chemotherapy in EGFR-positive NSCLC patients with brain metastasis

Abstract:Objective To evaluate the clinical efficacy and safety of Osimertinib combined with platinum-based chemotherapy in EGFR-positive non-small cell lung cancer(NSCLC)patients with brain metastasis Methods A total of 90 EGFR-positive NSCLC patients with brain metastasis admitted between January 2021 and December 2023 were selected and divided into the observation group(oral Osimertinib+platinum-based chemotherapy,n=45)and the control group(Osimertinib monotherapy,n=45)according to random number table method.The efficacy indicators[intracranial objective response rate(ORR),overall ORR],angiogenesis markers[vascular endothelial growth factor(VEGF),thymidine kinase levels],peripheral blood T lymphocyte subsets(CD4+,CD8+,CD4+/CD8+),serum tumor marker levels[carcinoembryonic antigen(CEA),carbohydrate antigen 50(CA50),carbohydrate antigen 125(CA125)],and adverse reactions were compared between the two groups.Results The intracranial ORR and overall ORR in the observation group were higher than those in the control group(P<0.05).After 3 and 6 treatment cycles,the observation group showed significantly lower levels of VEGF,TK1,CEA,CA50,CA125,and CD8+compared with the control group,while CD4+and CD4+/CD8+ratios were higher(P<0.01).During treatment,the incidence of grade III-IV adverse reactions was comparable between the two groups(P>0.05).Conclusion The combination of Osimertinib and Cisplatin+Pemetrexed chemotherapy can significantly improve the ORR of EGFR-positive NSCLC patients with brain metastasis compared with Osimertinib monotherapy,and can effectively inhibit angiogenesis,improve immune function,and reduce tumor marker levels,with controllable adverse reactions.
