Effect of early hyperbaric oxygen therapy and its impact on prognosis in patients with wake-up acute ischemic stroke

Abstract：Objective To observe clinical effect of early hyperbaric oxygen therapy and its prognosis in patients with wake-up acute ischemic stroke(WU-AIS).Methods A total of 80 patients with WU-AIS who didn't undergo intravenous thrombolytic therapy between November 2022 and December 2023 were selected.According to random number table method,they were divided into the observation group(n=40,hyperbaric oxygen therapy+conventional therapy)and the control group(n=40,conventional therapy).The levels of serum inflammatory factors[hypersensitive C reactive protein(hs-CRP),interleukin(IL)-6,tumor necrosis factor(TNF)-α]and oxidized low-density lipoprotein(ox-LDL)before and after treatment were compared between the two groups.The changes in scores of National Institutes of Health Stroke Scale(NIHSS)and Modified Rankin Scale(mRS)at different time points before and after treatment,and the occurrence of adverse events were recorded.Results After treatment,levels of serum hs-CRP,IL-6,TNF-α and ox-LDL in the observation group were lower than those in the control group(P<0.01).At 14 d and 30 d after treatment and at 3 months and 6 months after treatment,NIHSS scores in observation group were lower than those in control group(P<0.05).At 30 d after treatment and at 3 months and 6 months after treatment,percentages of patients with mRS score>2 points in the observation group were lower than those in the control group(P<0.05).There was no difference in the incidence of adverse events between the two groups(P>0.05).Conclusion Early hyperbaric oxygen therapy can effectively reduce inflammatory response,regulate lipid metabolism,relieve oxidative stress injury in brain tissues,promote recovery of nerve function and improve prognosis of patients with WU-AIS,which has certain safety.
