Impact of bedside bowel training on calcium and phosphorus metabolism,iron metabolism and quality of life in patients with constipation undergoing hemodialysis

Abstract:Objective To investigate the effects of bedside bowel training on calcium and phosphorus metabolism,hemoglobin(Hb),ferritin,transferrin saturation(TSAT),and quality of life in patients with constipation undergoing hemodialysis.Methods A total of 126 patients with constipation during hemodialysis who visited the hospital from February 2023 to February 2024 were selected and divided into the control group(n=63)and the observation group(n=63)by random number table method.The control group was given conventional intervention for constipation,while the observation group was combined with bedside bowel training on the basis of the control group.The curative effects,clinical symptoms[Patient Assessment of Constipation Symptoms(PAC-SYM)],calcium and phosphorus metabolism,disease-related indicators[potassium,intact parathyroid hormone(iPTH)],Hb,ferritin,TSAT,and quality of life[Patient Assessment of Constipation Quality of Life(PAC-QOL)]of the two groups were compared.Results The total effective rate of the observation group[90.48%(57/63)]was higher than that of the control group[74.60%(47/63)](P<0.05).After the intervention,the scores of fecal traits,rectal symptoms,abdominal symptoms and total score of PAC-SYM in the observation group were lower than those in the control group(P<0.05).After the intervention,the levels of phosphorus and iPTH in the observation group were lower than those in the control group(P<0.05).After the intervention,both Hb and TSAT in the observation group were increased compared with those before the intervention,while ferritin was decreased compared with that before the intervention.Moreover,Hb and TSAT in the observation group were higher than those in the control group,while ferritin was lower than that in the control group(P<0.05).After the intervention,the scores of physical discomfort,psychosocial discomfort,worry and anxiety,satisfaction and total score of PAC-QO in the observation group were lower than those in the control group(P<0.05).Conclusion Combined bedside bowel training can enhance the intervention effect on constipation patients undergoing hemodialysis,improve constipation symptoms,correct calcium and phosphorus metabolism disorders,iron metabolism,hyperparathyroidism,improve the state of malnutrition,and enhance the quality of life of patients.
