Analysis of the causes of misdiagnosis of histiocytic necrotizing lymphadenitis after multidisciplinary team consultation

Abstract:Objective To explore the causes of misdiagnosis in histiocytic necrotizing lymphadenitis(HNL)and to propose preventive measures.Methods A retrospective analysis was conducted on the clinical data of a patient with HNL that was diagnosed and treated using multidisciplinary team(MDT)consultation in October 2023.Results The patient presented with fever of unknown origin,accompanied by cervical lymph node enlargement.The clinical symptoms were atypical,and the tuberculin test was positive,which led to a misdiagnosis of lymph node tuberculosis.Experimental anti-tuberculosis treatment did not yield any improvement.The diagnosis and treatment process spanned half a year.Eventually,lymph node puncture and pathological examination confirmed the diagnosis.After treatment with Glucocorticoids,the symptoms were alleviated.Follow-up visits at 6 months after discharge showed no recurrence,indicating a good prognosis.Conclusion For patients with fever of unknown origin accompanied by superficial lymph node enlargement,HNL should be taken into consideration.It is essential to inquire about a detailed medical history and conduct a systematic physical examination to identify symptoms and signs with diagnostic and differential diagnostic value.Selecting appropriate lymph nodes for puncture biopsy is a crucial basis for a definitive diagnosis.
