A case of polycythemia vera complicated with multiple myeloma and literature review

Abstract:Objective To explore the characteristics of polycythemia vera(PV)complicated with multiple myeloma(MM)and to review relevant literature.Methods A retrospective analysis was conducted on the clinical data of one patient with PV complicated with MM admitted in July 2017.Result The patient,a 60-year-old male,presented with numbness in both fingertips.A blood routine test showed a red blood cell count of 8.57×1012/L,a hemoglobin of 227.70 g/L,and a hematocrit of 0.709.Bone marrow smear,bone marrow biopsy,and detection of driver genes associated with myeloproliferative neoplasms indicated PV.The survival prognostic score for PV was intermediate-risk.Interferon,Hydroxyurea and Ruxolitinib Phosphate were successively applied for treatment.11 months afterwards,the patient sought medical attention due to symptoms of bone pain and anemia.After completing bone marrow smear,bone marrow biopsy,and fluorescence in situ hybridization tests,the diagnosis was PV complicated with MM.The duration of missed diagnosis was 11 months.After diagnosis,five courses of chemotherapy with Bortezomib+Thalidomide+Dexamethasone were administered.The patient's bone pain symptoms were significantly alleviated,but there was no decrease in the number of bone marrow plasma cells.The therapeutic effect was evaluated as stable disease.He was lost to follow-up half a year afterwards.Conclusion PV complicated with MM is relatively rare in clinical practice.When patients with myeloproliferative neoplasms present with bone pain and anemia,this disease should be highly suspected.
